Our approach at Empower Psychiatry is to comprehensively evaluate patients with depression who are not responding to medications. We start with a thorough reassessment of the patient’s diagnosis, ensuring its accuracy and exploring any underlying conditions such as bipolar disorder, anxiety disorders, or personality disorders that could affect treatment outcomes. We conduct detailed clinical interviews to explore the patient’s full psychiatric history, co-occurring medical conditions, substance use, and psychosocial stressors. Additionally, we utilize standardized rating scales for depression to quantify the severity of symptoms and monitor changes over time. Reviewing the patient’s medication history is crucial, including dosages, adherence, side effects, and any previous responses to treatments.
We also consider non-pharmacological factors and interventions. Psychotherapy, particularly cognitive-behavioral therapy (CBT) can be highly effective for treatment-resistant depression and is integrated into our treatment plans if not already in place. We evaluate and address social and environmental factors, including support systems, lifestyle factors (such as sleep, exercise, and diet), and ongoing stressors.
